The Rehabilitation team at Reformed Church Home has been named a 2017 Post-Acute Care Partner with Robert Wood Johnson/Barnabas Health! Our services -- which include physical, occupational, and speech therapy available 7 days a week -- help short-term and long-term residents restore function and regain independence, confidence and optimal quality of life, especially those recovering from illness, injury or surgery.
Skilled Long-term Nursing provides 5-star-rated services for seniors with more complicated medical needs, and those who require more intensive nursing supervision plus help with activities of daily living, such as dressing, bathing and other personal care assistance.
Respite Care provides temporary furnished rooms in assisted living or skilled nursing, when available, along with all services available to long term residents. Respite stays are ideal for seniors who need short-term help after a hospital or rehab discharge, for a trial period before committing to a long-term move, or for coverage during a family vacation or caregiver break.
