Skilled Nursing Indoor Visitation Temporarily On Hold

Indoor Visitation Guidelines for Skilled Nursing Reformed Church Home

It is the policy of the Reformed Church Home to allow scheduled indoor visitation if no new facility-onset of COVID-19 cases in the last 14 days have been detected in accordance with the CDC guidance AND the facility is not currently conducting outbreak testing in accordance with CDC testing requirements. Indoor visits began on November 2, 2020, but placed on hold on November 4 due to identification of one COVID-positive case in a staff member as found during weekly testing. We reserve the right to cancel indoor visits as necessary per direction of the Department of Health.

Procedure: 

a)    Indoor Visitation times will be posted on SignUp Genius. Family members can create an account and sign up for a time to visit their loved one by visiting www.signupgenius.com. Scroll to bottom of this article to see complete sign up instructions. Time slots are available on a first come, first serve basis. 

b)     Indoor visits for skilled nursing residents may be scheduled for Monday, Tuesday, Friday and Saturday afternoons. (This applies to all residents unless the resident must be quarantined in the 14-day transitional unit after a new admission or hospital stay, or the resident is in the COVID wing.) Visits will be limited to thirty minutes per resident, per week in an effort to accommodate as many residents and families as possible. If time slots are available after all residents receive an initial appointment, then additional visits may be scheduled.  

c)    Reformed Church Home will honor each resident’s right to have and choose visitors and to make preferences. The activity staff will meet with each resident to determine who the resident wishes to visit with in person. During these visits, the activities staff will explain to the resident how visitation will work and what the resident can expect. 

d)    The visitation policy will be provided to residents, resident’s visitors, staff and others, as needed in writing, via the email chain used to convey COVID-19 information and policy changes.

e)    If a resident resides in a private room, the visit may take place in the resident room.  Residents who reside in shared accommodations, must meet their visitor in one of the three locations designated for such purposes:

  1. Second floor library
  2. Second floor living room
  3. Lobby front 

f)     Visitors will be provided with the visitation guidelines upon check in as well as information about the risks and dangers associated with COVID-19.  Each visitor and resident will be required to sign an informed consent form acknowledging they have received and agree to follow the guidelines. 

g)   Prior to the indoor visit, visitors will are strongly encouraged to have obtained the annual flu vaccine.  Prior to the visit, visitors are strongly encouraged to have a negative COVID-19 rapid test, which they can obtain from any medical provider in the community.

h)   Visitors are also required to complete a screening questionnaire/informed consent form upon arrival and have their temperature taken. Please see link below to a PDF version of this form so that you can print and complete before arrival to make check-in easier. Any visitor who has symptoms of any infectious disease or a temperature will not be permitted to visit with the resident. 

i)    Once cleared, visitors will be asked to go directly to their visitation destination.  Visitors are asked to remain in the visitation area the entire time. Visitors are asked to abstain from walking through the halls, visiting the dining rooms or nurses’ station. 

j)     Each resident is limited to no more than two visitors per visit.

i)     Residents must wear a face covering (surgical mask if supply is available); and visitors must wear a face covering or mask for the duration of the visit. 

j)     Visitors must restrict physical contact with the resident. 

k)    Visitors and residents are not able to eat or drink during the visits. 

l)     Residents having visitation in private rooms are asked to keep the room door open to enhance air circulation.

m)  At the conclusion of the visit, visitors will be asked to leave through the front door.  If the visitor needs to ride the elevator, visitors should refrain from getting on the elevator if another family is already present.

n)   The Public Restrooms on the first floor lobby will be available to visitors.  Visitors should refrain from using the resident’s bathroom.

o)    Visits may be rescheduled to patio visits or cancelled if the facility’s ongoing testing detects a positive staff member or resident.

Once you find the correct Sign Up:

You will see the available dates and times from which you can choose

Click on the “Sign Up” of your choice

Then, on the bottom of the page, click on “Submit and sign up”

Fill out all requested information

Then, click on “Sign up now”
